Who we are
The Fake Form Company is a brand of Mismatch Theory LLC, 8401 Mayland Dr, Ste A, Richmond, VA 23294, USA. Questions: forms@fakeformcompany.com.
What we collect
- If you just visit: nothing that identifies you. We count visits with Umami, a cookieless analytics tool that doesn’t build a profile of you. Like every website host, Cloudflare, which hosts this site, processes technical information such as your IP address and browser type to deliver pages and keep the site secure. This site sets no cookies.
- If you download a form: nothing. No email address is needed.
- If you file a Department Request: your email address, the department you chose, anything you write in the “not listed” box, and whether you ticked the merchandise box. Our email service, Kit, also records when you signed up and confirmed (with technical details such as your IP address, as proof that you asked to join) and whether our emails are opened and their links clicked.
How we use it
- To send the confirmation email and Form PT-345.
- To tell you when a department opens, or when the merchandise department opens if you asked, and, at most, to send the occasional bonus form.
- To count requests per department, which helps us decide which department opens first.
We don’t sell, rent or trade your information. The only companies that handle it are the ones that run this site: Kit (email), Cloudflare (hosting) and Umami (visit counts).
Where it’s kept, and for how long
Kit stores the list on its servers in the United States. Your details stay on the list until you unsubscribe or ask us to delete them. Unsubscribing stops the emails at once; to have your details erased entirely, email us.
Your choices
- Unsubscribe with the link at the bottom of every email.
- See, correct or delete your details: email forms@fakeformcompany.com. We’ll do it within 30 days.
- In the EU or UK: we rely on your consent, which you can withdraw at any time by unsubscribing. You can also complain to your local data protection authority.
Children
This site is office humor for adults and isn’t directed at children under 13. We don’t knowingly collect their information; if you think we have, email us and we’ll delete it.

Book links go to Amazon. They’re plain links, not affiliate links, and Amazon’s own privacy policy applies there.
Changes
If this policy changes, we’ll update this page and the date at the top. If a change affects how we use your email address, we’ll tell subscribers by email first.
